How does an automatic watch winder work?

Well, first, you need a watch that is automatic, which means it doesn't use batteries.

You place a cushion inside the bracelet of your watch and insert it into the provided slot. You turn the watch winder on and over the period in which it is in there. It will turn the watch clockwise and counter-clockwise with activates the watch internally and winds it.

Your other option, instead of a winder, is to turn the crown of the watch clockwise for about 4 minutes and then shake it profusely and you will watch it tick. You will have to do this daily, if not several times a day, to keep it properly set.

First of all never shake a watch profusely. I have many automatics and all it takes is a good wind of about 80 of your normal winds and the watch will do just fine. Wind it every day, once.
